2022 Shelby Community Health Assessment

The Shelby City Health Department is pleased to release the 2022 Shelby Community Health Assessment. This assessment provides an overview of health and health perceptions in the city of Shelby. It was developed with feedback from partners and stakeholders across the community and includes the results of the Healthy Shelby Community Health Survey that was conducted in late 2021. The health department and community partners will use this assessment to develop community health improvement strategies.

Healthy Shelby! Coalition Members Sought

Shelby City Health Department is seeking members of the community to join the Healthy Shelby! Coalition as a Community Member At Large. The Healthy Shelby! Coalition is a group of local partners who work together to improve health and wellness for residents in Shelby. Coalition members attend several meetings per year to share ideas and develop programs. Community Members At Large should have an interest in improving health and wellness in Shelby, live in Shelby, and be willing to participate in meetings. Applications are available by clicking the link below and must be submitted by Friday, January 7, 2022. Anyone with questions can contact the health department at (419) 342-5226.

Community Health Assessment 2020 Data – Preliminary Findings Released

Dear Community Member,

In 2020, Shelby City Health Department began the process of updating our community health assessment. This process involved collecting data on the health and well-being of Shelby residents from various sources in our community. While the COVID-19 pandemic delayed the completion of the community health assessment, we are pleased to share our preliminary data findings with you now. Along with our ongoing community health survey, this data will help us learn more about our community’s health so we can focus our public health programs on areas of need.

Community Health Survey 

As part of our community health assessment, SCHD is conducting a survey about health, healthcare, and quality of life in Shelby. Your feedback is an important part of this process! You’re invited to complete this survey and share your opinion with us. The survey is completely anonymous and takes less than 10 minutes to complete. Just click the blue box above this text to open the survey.

Print surveys will also be mailed to every household in Shelby in the December utility bill and are available at the following community locations:

  • City Hall/Shelby City Health Department
  • FISH Food Pantry
  • Marvin Memorial Library
  • Shelby Hospital
  • Shelby Senior Center
  • YMCA

Healthy Shelby! is a community coalition focused on improving the public’s health and reducing chronic disease through community-wide efforts. The coalition includes members from across the community, including healthcare organizations, social services, community-based organizations, the faith-based community, and the public.
